New research published in JAMA Network Open reveals that taking blood pressure medication at bedtime—rather than in the morning—can significantly improve both nighttime and daytime blood pressure control in patients with hypertension

In a randomized trial across 15 hospitals in China:

  • Patients who took their meds at night had better reductions in nighttime systolic and diastolic pressure
  • 79% achieved nighttime BP control vs. 69.8% in the morning group
  • Daytime and 24-hour BP control remained effective
  • No increase in adverse effects or nocturnal hypotension

This study supports the growing evidence behind chronotherapy—aligning medication timing with the body’s circadian rhythms to optimize outcomes.

Clinicians may consider bedtime dosing as a strategy to enhance cardiovascular protection, especially for patients with poor nocturnal BP control.
